Age Concern Hertfordshire, in partnership with Age Concern Dacorum, now offers a Carers' Support scheme countywide.

If you spend your days caring for a relative or friend you may feel you need some support, but don't know where to go.  Our Carers' Support scheme is aimed at older people who are carers.

Our Carers' Support workers can provide friendly, practical and emotional support.

     Age Concern Hertfordshire, in partnership with Age Concern Dacorum, now runs a county wide  Hospital Discharge and Independent Living Scheme.

    If you are an older person who has been recently been discharged from hospital or intermediate care, you may find our scheme helpful.
