Age Concern Hertfordshire has been successful in receiving funding from the Big Lottery Fund for the Advice Plus project.  This funding has enabled us to expand our Information and Advice schemes, which are designed to help older people, their carers and relatives to clarify options, obtain entitlements, access suitable services and resolve issues of concern.

Services available include:

  • Asian and Italian outreach
  • Advocacy in Care Homes

  • New Hospital Discharge and Independent Living SchemeNew Hospital Discharge and Independent Living Scheme

     Age Concern Hertfordshire, in partnership with Age Concern Dacorum, now runs a county wide  Hospital Discharge and Independent Living Scheme.

    If you are an older person who has been recently been discharged from hospital or intermediate care, you may find our scheme helpful.

    The scheme offers short term practical and emotional support to help you to regain your independence at home.  To find out more about the scheme, click here.
