New Projects for Age Concern Hertfordshire

Age Concern Hertfordshire has launched a number of new projects recently. Details follow:
- InTouch provides phone support to older people who are referred to Age Concern by statutory services for non-medical specialist support, which is vital to health and wellbeing.
- Healthwise has been set up to support older people who experience depression and/or the early stages of dementia. Currently running in Broxbourne, East Herts and Hertsmere.
- Mental Health Hospital Discharge offers short term practical support to older people going home after a stay in hospital. Aimed at older people with depression or early dementia.
- SWIFT supports older people in the community who have memory loss or who feel confused, anxious or depressed.
